THE GROVE ATHLETES ACADEMY
LIABILITY WAIVER, ASSUMPTION OF RISK, MEDICAL AUTHORIZATION & PROGRAM POLICIES
By registering your athlete for The Grove Athletes Academy, you (the parent or legal guardian) agree to all terms outlined in this document.

1. ASSUMPTION OF RISK

I understand and acknowledge that participation in athletic training, sport activities, games, and physical education programs involves inherent risks of injury, including but not limited to muscle strains, sprains, fractures, concussions, and other physical harm. I voluntarily assume all risks associated with my child's participation in The Grove Athletes Academy.

2. RELEASE OF LIABILITY
─────────────────────────────────────────
In consideration of my child's participation in The Grove Athletes Academy, I hereby release, discharge, and hold harmless The Grove Athletics, its coaches, staff, volunteers, agents, and facility partners from any and all claims, damages, injuries, losses, or liabilities arising out of or in connection with my child's participation in the program, whether caused by negligence or otherwise.

3. MEDICAL AUTHORIZATION

I authorize The Grove Athletics staff to seek emergency medical treatment for my child if I cannot be reached in an emergency. I agree to be responsible for all medical expenses incurred. I confirm that I have disclosed all relevant medical conditions, allergies, and medications on the registration form.

4. PHOTO & VIDEO CONSENT

I grant permission for The Grove Athletics to photograph or video my child during Academy sessions for use in promotional materials, social media, and the organization's website, unless I have indicated otherwise on the registration form.

5. CODE OF CONDUCT
─────────────────────────────────────────
I understand that my child is expected to:
• Respect coaches, staff, and fellow athletes
• Follow all safety rules and instructions
• Participate with effort, sportsmanship, and a positive attitude
• Refrain from bullying, aggressive behaviour, or disruptive conduct
The Grove Athletics reserves the right to remove any athlete from the program for repeated or serious violations of these expectations without a refund.

6. PROGRAM POLICIES
─────────────────────────────────────────
• Athletes must arrive on time for each session with appropriate athletic clothing and footwear.
• The Grove Athletics is not responsible for lost or stolen personal items.
• This registration is for a 4-week block consisting of two 60-minute sessions per week (8 total sessions).
• Refunds are not available once the block has started.
• In the event of a cancellation by The Grove Athletics, a full refund will be issued.

• I am the parent or legal guardian of the registered athlete.
• I have read and understand this entire document.
• I agree to all terms on behalf of myself and my child.
• I understand this is a legally binding agreement.
